DCMC Offers Online COVID-19 Screening Form

To accommodate increased testing for the coronavirus, Door County Medical Center (DCMC) has established an online COVID-19 screening form. 

Starting Oct. 13, patients who are concerned about exposure to or are experiencing symptoms of COVID-19 should visit dcmedical.org/covid19 to fill out the form. After submission, DCMC staff members will contact patients as soon as possible Monday-Friday, 8 am – 4:30 pm, to discuss their case and potentially schedule a test. The DCMC COVID-19 hotline is also still available; however, most callers who request testing will be asked to complete the online form.

Symptoms of COVID-19 include fever/chills, headache, sore throat, diarrhea, chest tightness, shortness of breath, any respiratory illness, body aches, nausea/vomiting, fatigue, cough, an alteration in smell or taste, nasal congestion and runny nose.
