Door County Community Foundation Awards Grant to Unity Hospice

Unity Hospice has been awarded a $3,000 Sustainability Grant from the Door County Community Foundation to provide end-of-life care for critically ill patients who have limited to no financial resources. As a 501(c)3, Unity relies on the financial contributions of community members and organizations like the Door County Community Foundation to help cover the end-of-life care costs for under-and uninsured patients, as well as unfunded services including medication delivery and grief support programs.

In 2015 alone, Unity provided $1.3 million in CommUnity Care to 272 patients, 20 residing in Door County. In addition, more than 7,300 children and adults benefited from unfunded grief support services valued at $337,000. Unity is the only provider of home-based palliative care for residents of Door County.

“The services provided by Unity are invaluable for community members who are coping with the effects of life-limiting and terminal illness,” said Polly Alberts, chair of the Door County Community Foundation.
