Evolve Physical Therapy Offers Services in Multiple Locations

Evolve Physical Therapy is now open in multiple locations. Evolve is a cash-based practice that uses flat-fee pricing and offers physical therapy care in a private setting. Additionally, no referral or prescription is needed, so clients may come directly to PT at their convenience.

Evolve provides individual outpatient services focusing on orthopedic and musculoskeletal health issues with other specialty options available. Evolve focuses on customized needs with a collaborative client-based approach. Locations include:  Juddville (private clinic), Sister Bay (Door County Fitness Studio), or in-home.

Evolve is an independently owned practice by Kelly Fagundes, licensed physical therapist. Fagundes has 14 years of clinical experience at premier health care organizations in Chicago, Madison, Green Bay and Door County.

Small group exercise classes are also conducted by Evolve to promote general wellness and help transition clients from rehab to community-based exercise programs. Classes are conducted at the newly opened Door County Fitness Studio.
