J-1 Visa Sign-Up Days at Ephraim Village Hall

On June 14 & 28, J-1 Social Security Card Sign-up Days will be held at the Ephraim Village Hall, 9996 Water Street. Representatives from the Social Security Administration will be in Door County to accept applications from J-1 Visa Students enrolled in the summer work & travel program this summer. New arrivals may apply without having to wait 10 days. If an employer is bringing two to four students in on the same day, the SSA representatives will be able to review them together at the same time.

Sign-up will be done by appointment only, and will be scheduled in the order received via email. Email Phil Berndt at [email protected] with the following information: your name, your business name, your phone number, preferred date, morning or afternoon preference, and number of students.

Students need to bring originals and provide photocopies of the following documents: Passport – copy of photo and facing page of information, and admission stamp; DS-2019 – Certificate of Eligibility for Exchange Visitor (J-1) Status; Completed SS-5 – Application for Social Security Card.
